Here you will find information and forms necessary to register as a client with Counseling SDC. We also house forms and worksheets frequently used in session and for homework here.
This is just for easy access.
If you are new, please feel free to fill out these intake forms before our first session. These forms can also be filled out during the first session.
If you feel that you would like to collaborate your session with another provider, simply fill out the following form.

Access your Client Portal
Once you select this option a link will be sent to your email from Simple Practice. This is our HIPAA compliant platform for managing health information. Please follow the instructions from Simple Practice to access your Client Portal account.
